The short answer is: lots of trial and error. I have to make an educated guess where to position myself, since I can't look through the camera and be in front of the camera at the same time. :-) It took 94 tries to get that shot. In contrast, it only took 7 or so frames to capture the shot of T!
On top of adjusting my body position after I broke the pose to examine each take, I held the camera's cable release in my left hand and my flash in my right hand. I had to manually pop the flash a split second after I tripped the shutter, because the only flash I have where I can adjust the power is not made for a Canon camera.
